SLIDING FEE DISCOUNT PROGRAM
Iliuliuk Family and Health Services, Inc. (IFHS) has a board-approved policy of providing medical care regardless of a patient’s ability to pay. In support of this policy, IFHS has developed a sliding discount (“discount”) schedule based on household income and household size that applies to all required and additional health services within the Health Resources and Services Administration (HRSA) approved scope of project. In order to facilitate patient access and utilization, IFHS staff will make patients aware of the Sliding Fee Discount Program by utilizing a variety of methods, languages, and literacy levels.
IFHS has a uniform fee schedule based on regional fee analyzer data reasonably adjusted for local costs. Eligible patients will receive discounts on out of pocket costs after third party payers are billed, based on their household income and household size.
IFHS requires that, at a minimum, patients requesting discounts initially provide the name, relationship to patient, age, gender, date of birth, monthly income and employer of every individual in the household, and sign a statement certifying this information is correct. Patients who present without the necessary proof of income information will be given a 30-day grace period to turn in information and qualify for a discount. Patients who chose not to provide proof of eligibility will not be eligible for any discounts. IFHS will review eligibility for each active patient at the first encounter greater than one (1) year after the last review.

Definition of Household:
All members of a household who are related and pooling financial resources are counted as one family if the arrangements are considered permanent and support greater than room and board is provided. Unrelated members of a household who are supporting one another financially are considered one family.
The Schedule of Medical Discounts based on the Alaska State poverty level table. This table is updated annually. IFHS will maintain the most current poverty level table in order to determine patient eligibility for discounts.

- Tax returns are the preferred documentation of income;
- Pay stubs from the last three months;
- Letters of income verification from the employer(s);
- Receipts from self-employment after deductions for normal operating expenses;
- Regular payments through public assistance, social security, longevity, unemployment, strike benefits, military allotments, disability, rental income, regular support from an absent family member or someone not living in the household (includes child support), government or private pensions, and regular insurance or annuity payments;
- Income from dividends (including permanent fund), interest, rents royalties, or income from estates or trusts.

- 100.1%-133% of Federal Poverty Guidelines for Alaska
- 133.1%-166% of Federal Poverty Guidelines for Alaska
- 166.1%-200% of Federal Poverty Guidelines for Alaska
- >200% of Federal Poverty Guidelines for Alaska
The following charges are excluded from eligibility for discount:
- Dispensary stat fees.
- Dispensary restock fees.
- Prescription charges for Viagra or Cialis.
- Plan B more than once per month.
- Any cosmetic procedures.
- Any non-medically necessary procedures.
